Why Family Connects
The postpartum period is an important time for recovery, addressing delivery complications, managing infant care and setting the stage for long-term health and well-being.
Through Family Connects, all eligible families have the opportunity for a trained, registered nurse to visit them at home during their newborn’s first few weeks of life. Beginning at three weeks of age, the program provides between one and three nurse home visits to every family with a newborn regardless of income or demographic risk. Using a tested screening tool, the nurse measures newborn and maternal health and assesses strengths and needs to link the family to community resources.
Helping parents connect with their infants helps them for life and community-wide eligibility is the only route to population change.

Research has shown that when families participate in Family Connects, mothers feel less anxious, parents learn about quality child care options, and babies need less emergency care at hospitals. That is good for babies, families and the whole community.

Are you eligible for a free nurse home visit?
To be eligible, you must:
- Have delivered your baby at Ascension Seton Main on 38th St or St. David’s South on Ben White Blvd, OR
- Be eligible for WIC benefits
